Job Title:  Nurse

Classification:  Non-Exempt

Definition and Basic Function:  Provide oversight of comprehensive and holistic care that will include direct care to individuals and initiate services designed to promote health, prevent illness, and ensure general well-being in keeping with the overall faith-based and spiritual vision and mission of Jessica & Friends Community including nutrition, exercise, and dealing with other individuals.

Accountability:  Executive Director of Operations


Essential Functions for the Individuals we Serve:

  1. Promote and maintain health, safety, and welfare
  2. Ensure that the medical care and related documentation is accurate and complete
  3. Ensure that medical appointments are completed as well as follow-up care from the appointments
  4. Promote and support healthy choices to advance and maintain wellness
  5. Plan, coordinate, implement, and evaluate holistic care plans to maintain physical, mental, emotional, and spiritual well-being
  6. Establish and perform monthly medical assessments
  7. Assist staff throughout illnesses, injuries, or health related issues as needed
  8. Monitor medication labels and review and update medical administration records on a monthly basis
  9. Attend medical appointments as needed
  10. Initiate and supervise monthly in-service training for staff regarding diagnoses, health, medical equipment, and medication issues
  11. Ensure medical and adaptive equipment are functional
  12. Serve as primary liaison to families, Program Managers, and staff when individuals are hospitalized and ensure follow up after discharge
  13. Review safety plans as needed
  14. Create and modify charts or documentation as needed
  15. Assist in creating and modifying Refusal for Treatment Plans as needed
  16. Complete monthly quality assurance checks on medical equipment and supplies (AED, first aid supplies, over the counter medications, etc.)
  17. Assist with personal care needs as needed.  


  1. Works with community partners (Church of the Open Door) on periodic health screenings and trainings.
  2. Conduct quarterly training to staff & caregivers related to our individuals & their own aging health care needs.
  3. Communicate with Program Managers and other staff through appropriate means.
  4. Attend program meetings as needed
  5. Serve on committees as designated
  6. Serve as emergency on-call for medical issues of individuals
  7. Participate in at least twenty-four hours of training each fiscal year as required by the regulations.
  8. All other duties as assigned.

Work Schedule:  Full-time:  Generally Monday through Friday 8:00 a.m. - 4:30 p.m. but will vary depending on the needs of the individuals.  Will include evening and weekend work as needed (will be offset by time off during the week). Part-time: Generally 15 - 20 hours per week to be determined by the needs of the individuals.

Job Requirements:  Must be licensed as a practical nurse or registered nurse in Pennsylvania.  Must also have a valid driver's license, valid auto insurance, and valid car registration.   Must have at least two years of driving experience to transport individuals using personal vehicle.  Must submit to a criminal background check, a driver's record check, an initial physical, and a substance screening.  Additional education or experience in human services, especially in the area of Intellectual Disabilities/Autism is preferred.  Post licensure experience is preferred. Evidence of continuing education in line with community, state, and federal regulations is required.
